The MilVet Jr. Program is a year-round initiative that hopes to engage and inspire military-connected and community children through hands-on learning, service projects, and

creative activities. To participate, families are asked to contribute a small donation fee of $50 at sign-up , which includes a MilVet Jr. t-shirt and a MilVet Jr. backpack for each child.

This program will be led by Mayra Lewis, a Certified Behavior Technician and current college student working toward a degree in elementary education. Together, with the support of MilVet and the local community, we hope to encourage service, kindness, and leadership within our younger volunteers.

I, the undersigned parent or legal guardian of the child named above, give permission for my child to participate in the MilVet Jr. Program, which includes monthly on-site and off site activities. I understand that all reasonable precautions will be taken to ensure the safety and well-being of my child. I acknowledge that participation in any activity involves some level of risk, including but not limited to trips, falls, minor injuries, or other unforeseen events. I agree to hold harmless and release MilVet, its staff, volunteers, and affiliates from any liability, claim, or responsibility for any injury, illness, or loss sustained during program activities, except in cases of gross negligence or misconduct. I confirm that my child is in good health and able to participate. I will inform the program coordinator of any medical or behavioral conditions that should be noted. I agree to supervise my child at all meetings and events.
